Comparison of mean systemic pressure in patients with acute circulatory failure receiving passive leg raising vs. pneumatic leg compression

چکیده

Background: Driving pressure of venous return (VR) is determined by a gradient between mean systemic (Pms) and central (CVP). While passive leg raising (PLR) pneumatic compression PC (PC) can increase VR, no study has explored the effects these two procedures on Pms VR-related hemodynamic variables. Methods: Forty patients with acute circulatory failure were enrolled in this analysis. All obtained both PLR PC, measured for Pms, CVP, arterial (MAP), cardiac output (CO), VR resistance (RVR), vascular (SVR) at baseline immediately after procedures. To minimize carry over effect, divided 2 groups based procedure sequence which 1) receiving first then (PLR-first), 2) (PC-first). Both waited washout period before performing second procedure. Primary outcome was difference Secondary differences MAP, CO, RVR, SVR Results: No found characteristics effect observed patients. Compared baseline, significantly increased CO. (9.0±2.3 vs 4.8±1.7 mmHg, p<0.001), CVP (4.5±1.2 vs. 1.6±0.7 MAP (22.5±5.6 14.4±5.0 CO (1.5±0.5 0.5±0.2 L/min, p<0.001) more than PC. However, also RVR (16 ± 27.2 dyn.s/cm5, p=0.001) (78.4 7.2 but group. Conclusion: Among failure,

Passive leg raising and compression stockings: a note of caution

In a recent editorial in Critical Care, Monnet and Teboul emphasize the value of passive leg raising (PLR) as a reliable bedside indicator of fluid responsiveness in critically ill patients [1]. This is because PLR induces changes in venous return, regardless of the mode of ventilation or the underlying cardiac arrhythmias. In their eloquently written editorial, they provide five practical rule...

Effects of Passive Leg Raising And Volume Expansion On Mean Systemic Pressure And Venous Return in Shock in Humans

INTRODUCTION The aim of this study was to assess how mean systemic pressure (Psm) and resistance to venous return (Rvr) behave during passive leg raising (PLR) in cases of fluid responsiveness and fluid unresponsiveness. METHOD In 30 patients with an acute circulatory failure, in order to estimate the venous return curve, we constructed the regression line between pairs of cardiac index (CI) ...

Effect of intermittent pneumatic leg compression on intracranial pressure in brain-injured patients.

OBJECTIVE To evaluate the effect of intermittent pneumatic leg compression on intracranial pressure and cerebral perfusion pressure in brain-injured patients. Passive leg raising predicts fluid responsiveness in the critically ill.

OBJECTIVE Passive leg raising (PLR) represents a "self-volume challenge" that could predict fluid response and might be useful when the respiratory variation of stroke volume cannot be used for that purpose. We hypothesized that the hemodynamic response to PLR predicts fluid responsiveness in mechanically ventilated patients. DESIGN Prospective study. SETTING Medical intensive care unit of ...

Passive Leg-Raising and Prediction of Fluid Responsiveness: Systematic Review.

Fluid boluses are often administered with the aim of improving tissue hypoperfusion in shock. However, only approximately 50% of patients respond to fluid administration with a clinically significant increase in stroke volume. Fluid overload can exacerbate pulmonary edema, precipitate respiratory failure, and prolong mechanical ventilation. Therefore, it is important to predict which hemodynami...
